The Social Services Coordinator (SSC) is a member of an interdisciplinary team that also includes a Registered Nurse, Nurse Practitioner and Team Assistant. The SSC is responsible for assessing, delivering, coordinating, providing case management and monitoring and evaluating the psycho-social services needs of the member in order to live as independently as possible in the community.
