   <description>A.	Assessment:  Assesses resident needs and develops a plan of care.
1.	Performs nursing physical assessment and completes data base within one hour of admission.
2.	Identifies and documents, according to \policy, current resident problems, symptoms and behavioral changes.
3.	Reviews data obtained and documented by other nursing members.
4.	Discusses abnormal or unusual assessment findings with ADON/ Supervisor as needed.
5.	Assesses the resident learning needs related to present health status

B.	Planning:  Develops an individual resident problem list and a resident plan of care to assure assessed needs are addressed.
	
1.	Completed MDS by 14th day of admission.
2.	Develops a plan of care for assigned resident and formulates written care plan, and seeks assistance as need by 21st day of admission and when a permanent change occurs in resident status.
3.	Communicates pertinent information regarding plan of care to other nursing personnel.
4.	Completes MMDS on a monthly basis.

C.	Implementation:  Demonstrates the basic skill and judgment necessary to implement physician’s orders, nursing interventions and procedures as needed for the care of the resident.
	
1.	Performs nursing procedures common to the unit.
2.	Implements the medical plan of care as delegated.
3.	Follows established standards of care/policies and uses safety practices in the provision of resident care.
4.	Demonstrates basic knowledge of usual disease processes.
5.	Periodically makes complete resident rounds and modifies care to meet changing resident needs.
6.	Administers medication according to Policy/Procedure and is aware of their actions, doses, side effects.
7.	Communicates change in resident status to supervisor, physician, and family as appropriate in a timely manner.
8.	Documents administration of medications, nursing interventions, nursing observation, and performance of treatments according to policy and on appropriate forms.
9.	Documents, according to policy, discharge instructions and/or nursing transfer summary.
