Key Job Responsibilities

Prepares and cooks meals for patients daily, monitors patient menus, sorts, tallies, and completes meal tally sheets daily.

Audits patient refrigerators in nursing station against par sheets and restocks necessary items each day

Delivers fresh ice and water pitchers to the patient wings each day

Utilizing the Food/Supplies Delivery Van, the Dietary Worker will deliver food trays to patients at the Dayton and Butler/Warren In-Patient Units at agreed upon scheduled times.

Schedule

Hours will vary within the hours between 6a to 10:30p. This position will be a fill-in for other Culinary Specialists during time-off and therefore the schedule will not be fixed. There will be some weekend work required.

SUMMARY

This position will consist of two functions; pick-up & delivery of donated items such as furniture and customer service for the Hospice of Dayton Heirlooms Shoppes.

Pick-up/Delivery Truck Driver - Must have a safe driving record, auto insurance in your name and a valid driver's license. This is a physical position; requiring pick-up of sometimes large, bulky and heavy items such as furniture.

Resale Shop Associate - Provide managerial oversight for all volunteers assigned to the Shop; they are responsible for maintaining accurate sales records of daily sales and are expected to analyze daily, weekly and monthly sales data to make recommendations regarding store hours and shop inventory. Co-managers are also expected to assess whether or not to accept offered items; make arrangements to have donated furniture picked up, inventoried, priced, and sold at the Shop. All items that are deemed not up to the standards of the HOD Shop are to be delivered by the co-managers to one of the area’s nonprofit thrift stores.

Job Summary/Purpose:

Hospice of Dayton patients will receive quality respiratory care and coordination of hospice services in a manner that meets specific patients need, HOD standards, Medicare Conditions of Participation (COP) and achieves desired outcome of comfortable and peaceful death for patients. Respiratory visits will be prioritized on patients’ emergent needs, diagnosis, and complex equipment needs. Respiratory patient visits are required for patients with a diagnosis of ALS, Emphysema, Pulmonary Fibrosis, and Respiratory Failure. Visits for patients with lung cancer will be based on nurse request or respiratory complexity of patient’s care. Complex needs include C-PAP, BiPAP, Tracheostomy, chest tubes and high flow O2.

Key Job Responsibilities:

Quality of care and coordination of services

The respiratory therapist makes 15-18 patient contacts per week thereby ensuring sufficient frequency to evaluate the status of the patient’s condition and provide quality patient care

Respiratory assessments are complete including lung sounds, vital signs, pulse oximeter and medication review and documented in the EMR at point of care ensuring quality patient care and symptom management.

Input is provided to the respiratory plan of care and is individualized and consistently updated to reflect assessment findings documented in the EMR (clinical notes) at point of care. Staff adds to plan of care based on respiratory therapist’s suggestion.

Coordination of services and care are met by means of an interdisciplinary team and documented in the EMR at point of care with a satisfaction standard of 100% ensuring quality patient care.

Visits are consistently adjusted to meet changes in respiratory status and documented in clinical notes at point of care to ensure quality patient care and effective communication.

Unit Coordinator - Hospice of Butler/Warren Counties (Butler/Warren County)

SUMMARY

This position is responsible for inpatient unit communications between staff and patients, and facilitation of the admission and transfer process for patients entering and leaving the Care Center and maintaining medical records. Retrieves charts from HIM open file area when patients are transferred into the HCC.

After a transfer, discharge, or death prepares the chart for return to HIM.

Summary: Provide assistance to directors, service line teams, and other clinical staff. Compiles, verifies, types, and files active/inactive medical records by performing the duties listed below. The HIM Team Assistant II is committed to the mission, purpose, philosophy, and goals of the hospice concept of care.

1.Verify with Assessment Nurse/Liaison and inputs information relative to patient including the status of the patient, primary/secondary diagnosis, etc. and process Hospice Care Center documentation within 24 – 48 hrs. of receiving paperwork with a satisfaction rate of 90% to obtain compliance with all regulatory standards.

2.Fax/Mail Initial Certification Order, DNR, Care Plan for physician signature within 24 hrs. of receiving paperwork with a 95% satisfaction rate. Must obtain all signatures before submission of claims according to Medicare COP.

3.Follow-up every five days for paperwork not received from physician by phone contact, fax, and/or mail with a 90% satisfaction rate to remain in compliance with regulatory standards and HOD policy.

4.Prepare/assemble patient medical records for newly admitted patients the same day paperwork is received. Maintain complete records of active and inactive clients for accurate records according to regulatory standards and HOD policies. Files information obtained from outside agencies and/or information received from HOD staff to patient record, according to chart order to ensure all individuals have current information on their patients.

5. Receives and tracks death certificates for Medical Director Signature on a daily basis to assist funeral homes and care givers with needed documentation. Cremation death certificates signed within 1-2 hrs of receiving if not sooner. Burial death certificates within 72 hrs of receiving paperwork with a 95% satisfaction rate to ensure compliance with HOD standards and serving the public in a timely manner.

6.Direct/Indirect contact with HOD customers on a daily basis ensuring open communication to satisfy patient and caregiver needs.

7.Maintains confidentiality of medical records and any information for the medical record to comply with HOD policies and HIPAA.

8.Operates computer to enter, correct, and retrieve data via hard copy, HIM E-Mail, or direct phone conversations for compliance with all regulatory standards.

9.Assist in completing/processing with request for information from outside correspondence within 10-15 working days of receiving request to ensure adherence to HOD policy.
