   <description>&lt;p&gt;	This position will provide coverage for weekends and vacations. PRN employees work as needed and are not benefit eligible.&lt;/p&gt;&lt;p&gt;	&amp;nbsp;&lt;/p&gt;&lt;p&gt;	&lt;span style="text-decoration: underline"&gt;Job Summary/Purpose:&lt;/span&gt;&lt;/p&gt;&lt;p&gt;	Hospice of Dayton patients will receive quality respiratory care and coordination of hospice services in a manner that meets specific patients need, HOD standards, Medicare Conditions of Participation (COP) and achieves desired outcome of comfortable and peaceful death for patients. 	Flex RNs will be cross-trained to work in another department or position in addition to their primary role. In order to maintain familiarity with the new role the RN must work at least once every other month in the additional area.&lt;/p&gt;

The HIM Team Assistant II is committed to the mission, purpose, philosophy, and goals of the hospice concept of care.&lt;br&gt;&lt;br&gt;&lt;br&gt;1.Verify with Assessment Nurse/Liaison and inputs information relative to patient including the status of the patient, primary/secondary diagnosis, etc. and process Hospice Care Center documentation within 24 &amp;ndash; 48  hrs. of receiving paperwork with a satisfaction rate of 90% to obtain compliance with all regulatory standards.&lt;br&gt;&lt;br&gt;2.Fax/Mail Initial Certification Order, DNR, Care Plan for physician signature within 24 hrs. of receiving paperwork with a 95% satisfaction rate. Must obtain all signatures before submission of claims according to Medicare COP.&lt;br&gt;&lt;br&gt;3.Follow-up every five days for paperwork not received from physician by phone contact, fax, and/or mail with a 90% satisfaction rate to remain in compliance with regulatory standards and HOD policy.&lt;br&gt;&lt;br&gt;4.Prepare/assemble patient medical records for newly admitted patients the same day paperwork is received.  Maintain complete records of active and inactive clients for  accurate  records according to regulatory standards and HOD policies. Files information obtained from outside agencies and/or information received from HOD staff to patient record, according to chart order to ensure all individuals have current information on their patients. &lt;br&gt;&lt;br&gt;5. Receives and tracks death certificates for Medical Director Signature on a daily basis to assist funeral homes and care givers with needed documentation.  Cremation death certificates signed within 1-2 hrs of receiving if not sooner.  Burial death certificates within 72 hrs of receiving paperwork with a 95% satisfaction rate to ensure compliance with HOD standards and serving the public in a timely manner. &lt;br&gt;&lt;br&gt;6.Direct/Indirect contact with HOD customers on a daily basis ensuring open communication to satisfy patient and caregiver needs.&lt;br&gt;&lt;br&gt;7.Maintains confidentiality of medical records and any information for the medical record to comply with HOD policies and HIPAA. &lt;br&gt;&lt;br&gt;8.Operates computer to enter, correct, and retrieve data via hard copy, HIM E-Mail, or direct phone conversations for compliance with all regulatory standards.&lt;br&gt;&lt;br&gt;9.Assist in completing/processing with request for information from outside correspondence within 10-15 working days of receiving request to ensure adherence to HOD policy.&lt;br&gt;&lt;br&gt;10.Locates and signs out medical records to requesting staff and prepare medical records for patients transferring to the Care Center at time of request for continuity of care.&lt;br&gt;&lt;br&gt;11.Assists in chart audits as needed for compliance with all regulatory standards.&lt;br&gt;&lt;br&gt;12.Assists volunteers and Light Duty staff  involved in daily Health Information Management duties to help manage complia</description>
